1. First of all, we are going to make a nice background. Open a new document. Lets say 350 * 150 pixels.

2. Start brushing on the new document, try to give it a bit Glass look. I ended up with this:

3. Give the background some color. Do this by selecting Layer > new adjustment layer > color balance. I used this settings:

4. Again, make new adjustment layer, but this time Brightness / contrast. Set both to +20.  

5. After this we make a border around the background. Make a new layer, select all, Edit > stroke: Make it 3px / color doesn't matter. Go to blending options and select gradient fill, choose one you like most than stroke, color black, 1px. Now, make a second layer, select all again. Edit > stroke. This time make a 1px / black border. It should look like this now: (Notice that I put the border layers below the color adjustment to give it a bit the same color).

6. Now, here come the trickiest part of signature. First flatten the image to make it a solid background (Layer > flatten image). Notice that the new background is locked. Copy the locked layer by pressing ctrl+J. Now delete the locked layer. We gonna flip the background now to make it look 3d. First of all, we need more space, so we go to image > canvas size. Set the cm to pixels and make the image like 600 * 600 pixels (We can slice it to the needed size after we are done)

7. To flip the background, go to Edit > transform > perspective. Pull the lower right square more to the right to strech the bottom of the background. Now you can play a bit with free transform and transform > scew untill youget what you want. I got this:

8. It's time to get a render now. Make sure the render you gonna use is cut out good. You don't want to spoile your sig with lost pixels after you done. After opened your render, drag it onto the background. (Before you do this you might want to ad some changes to the render. I reduced the colors a bit with saturation, duplicate the render for some stronger details and put the same color-overlay I used for the background, to get almost the same color as I have on my background. Last, but not least, I gave the render proper size).

9. Place render in the middle of the background (as top-layer) and make a selection on the legs like this:

10. Now go to select > feather and set the radius to 15 pixels. After this hit delete once. It now looks like the render is popping out of the background.

11. To make sure all the pixels underneath the background are gone, make a new, huge selection that cover the former area of the feathered render and hit delete once more.

12. That leaves us with only some text to add. You can choose to put the text on the background before transform it. But sometimes it's gonna be unreadable after, so I go for a handwritting look, and use the color of the girls shirt. After I finished my text I will turn it slightly to the right to make it equel to the front-line of the 3d background (ctrl + t for free transform). Now duplicate the layer. Go to edit > transform > flip vertical and drag the text right under the original.
Note: Because you vertically flipped the text you have to rotate it again (ctrl + t) to get it in exact the same position as the original text. Set opacity to 50%.

13. All we have to do now is make a selection around signature as tight as possible with Rectangular Marquee tool (M) and go to Image > Crop. This will make the image size it has to be.

14. Now, we are finished. Make sure to save the file as .gif or .png. Png files gives a WAY better results, but...are only showing transparent in Mozilla Firefox and NOT in IE. Most on you guys will probably save it in GIF format.
